I believe most are typically based on common law fraud, and usually there's some kind of flat fine and the the rest is based off provable damages to other parties, rather than the amount of profit.

An annual screening mammogram is automatically covered. If it's a "diagnostic" mammogram following up some specific issue or complaint, then it would then fall into whatever normal policy rules you have with your insurance company for imaging tests. That's more likely the case here since this person had to go on to have an ultrasound as well.
If you have insurance but get billed for an annual screening mammogram, it's possible it was coded improperly by billers and you should definitely check up on that before paying.

No, the average internet download speed in the United State is about 171 Mbps. Though disclaimer, I'm not sure of the exact reliability of that number, different sources are reporting quite a range of speeds, though I don't see any under 100 Mbps average and I see many reporting well above this. You'd also have to consider median vs average since people with fiber sitting at gigabit speeds may be dragging that number up, median may be lower.

There are certainly some areas, especially rural, that struggle though. And upload speed is often much worse unless you have fiber. Major cities are definitely getting much better than 10 Mbps down though.

They're not even really available anymore. There are some but they're more meant to be directly sold to businesses, and often lack features in addition to costing much more. Easiest thing to do is get a smart TV and just don't let it connect to the internet.
